Safe employers are "preoccupied with the possibility of failure"

Employers that view every incident investigation as an opportunity to test their safety systems for deficiencies - as opposed to blaming workers - are more likely to "detect, contain and bounce back" from human error, Australian OHS lawyer Michael Tooma has told a US safety conference.
