Category 3 charges follow fire; Fatality findings released

A PCBU has been charged with category 3 WHS breaches after a workplace fire knocked out the communication and air-supply systems needed for such an emergency. Meanwhile, an investigation into a worker's death has found a risk assessment was not undertaken for the task that killed him.
