SA and Vic employers fined for confined space flaws, safety breaches

A South Australian employer that failed to properly maintain a machine's interlock switches (which were ultimately fixed for $1400), or ensure spotters kept confined-space workers in sight, has been fined $70,000. Also in this article, five Victorian entities have been fined for safety breaches.
