Work vehicle unroadworthy for 20 years, bystander killed

The Victorian Coroner has stressed the importance of following manufacturers' instructions for maintenance tasks - and made recommendations to WorkSafe and the National Heavy Vehicle Regulator - after an axle broke off a prime mover and struck and killed a contractor.
